The employer did not ensure that the side rails of the extension ladder used to access the roof extended above the roof line a minimum of 36 inches. The ladder extended approximately 26 inches above the roof line.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. Location: At or near 7411 Sieloff Dr., Hazelwood, MO 63042. The employer did not ensure that the ladder was set up at the proper angle. The horiztonal distance from the foot the the top support was calculated to be 7 ft. and the required distance should have been approximately 4 1/2 ft.
