29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) In Shop #62, Section #8, 9, 10; maintenance activities were attempted while the machine was still running where jars were traveling below employee on a platform. This exposed employees to burn injuries if the bottles jammed or made contact with the employees.
