29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: 1) 1031 Building Railroad Platform -On or about 3/13/12, employees were required to operate forklifts and walk next to the leading edge of an unprotected railroad platform, four and a half feet above ground level. Employees were thereby exposed to the hazards associated with falls.

29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board. 1) 1001 Building Furnace "E"- On or about 3/13/12, employees were required to work from a platform next to a furnace of molten metal. The platform was not provided with a standard railing system. Employees were thereby exposed to fall hazards (from the platform into the unprotected furnace).

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; or (a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS) 1) 1001 Building Maintenance Shop-On or about 03/13/12, employees were required to work near a compressed gas storage area which contained oxygen and acetylene cylinders stored adjacent to each other. Employees were thereby exposed to the hazards associated with improperly stored gas cylinders.
