29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. Employees roofing a three story apartment building were not protected from falling by properly used personal fall arrest systems or methods. a) Three employees were wearing harnesses that were not connected to lifelines. b) In three instances more than one lifeline was connected to a single anchor point. c) In four instances employees were connected to lifelines that would allow an employee to fall farther than 6' or to strike an object below the eave edge due to their excessive length. Four Seasons Roofing and Remodeling LLC was cited previously for violations of this Occupational Safety and Health Standard or and equivalent Standard, 1926.501(b)(10), which was contained in OSHA Inspection #312553977, Citation #1, Item #4a, which was affirmed as a Final Order on 2/1/10 with respect to a workplace located at 3129 Hwy 70, Eagle River, WI 54521.
