29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:    (a)  An injury that occurred on or about January 16, 2012 where an employee had a foreign body in his eye and had to seek medical treatment was not entered on the OSHA 300 log within 7 calendar days of receiving information that a recordable injury or illness occurred.

1910.133(a)(1): 1 29 CFR  1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:        (a) 11000 Northwest 45 Highway, Parkville, Missouri:  An employee used a table saw and was not protected by safety glasses with side shields when sawing wood.  On or about January 16, 2012, an employee received an eye injury when operating a table saw.
