29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about March 13, 2012, during construction activities including but not limited to roofing work on a low sloped roof, the employer did not ensure employees were protected from fall hazards approximately 24 feet by ensuring an effective warning line and safety monitor system was implemented and enforced.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: On or about March 13, 2012, during construction activities including but not limited to roofing work on a low sloped roof, the employer did not ensure a warning line was properly erected 6 feet from the roof edge exposing employees to an approximately 24 feet fall hazard.
