29 CFR 1910.305(g)(2)(iii):  Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a)  On or about March 13, 2012, and at times prior, employees were exposed to shock and burn hazards from cords that had the insulation pulled back from plugs on the following peices of equipment:     1) order picker recharger plug on the EXIDE XI System Type G, Model G1-12-550, Serial Number UG29814, in the receiving area.  2) JLG Electric ladder 12SP, Serial Number 0900030026, hand marking "#0".  3) Tennant 5680 Zamboni (floor polisher) stored in the janitor's closet. 29 CFR 1910.303(g)(1)(ii):  The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  (a)  On or about March 13, 2012, and at times prior, items were stored in front of electrical boxes, panels and switches, inhibiting access in case of emergency in the following locations:  1)  the computer room on the south side of the building on west end,   2)  the offices on the north side of the store,  3)  the janitor's closet on the south side of the store.  4)  the electrical room on the south side of the building.
