29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains were not enclosed and were less than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a) On or about March 13, 2012 and at times prior thereto, employees were exposed to amputation while working around the unguarded chain and sprocket on the log transfer table for the large peeler. b) On or about March 13, 2012 and at times prior thereto, employees were exposed to an amputation while using the blade sharpener.

The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) On or about March 13, 2012 and at times prior thereto, employees were performing maintenance on equipment without an energy control program.
