29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Auddino's Italian Bakery, 1490 Clara Avenue, Columbus Ohio establishment - As of and prior to March 14, 2012, an electrical power supply cord entering an opening in a control panel for the overhead dry proofer was not protected from abrasion. This condition poses an increased risk of electrocution for employees operating this equipment.

29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. Auddino's Italian Bakery, 1490 Clara Avenue, Columbus Ohio establishment - As of and prior to March 14, 2012, specific procedures for the control of hazardous energy (lockout/tagout) did not include verification of control measures. This step was left out of procedures for the Overhead Dry Proofer, Mixers and Polin Ovens. Employees perform covered duties when cleaning equipment in the production area on a routine and daily basis.
