29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    a) Building- On or about March 15, 2012 - During roof installation, employees were installing roof decking on a 7 on 12 pitched roof deck without properly utilizing conventional fall protection, CSHO observed employees working on the roof without safety harnesses connected to a lifeline. Employees were exposed to a fall hazard of 22 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a) Building- On or about March 15, 2012 - Aluminum Extension Ladder used by employees to gain access to the roof was not extended above the landing a minimum of three feet, ladder was only extended approximately 16 inches. Employees were exposed to a fall hazard of 22 feet.
