OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by/crushed while performing lifting operations utilizing a close proximity lifting magnet:    On or about March 12, 2012, the employer allowed a close proximity lifting magnet to be put into operation without ensuring that the employees were instructed on the safe operations of below-the-hook lifting devices, such as close proximity lifting magnets.  Employees were not instructed on, but not limited to, the following:    1.	 Instructions in any special operations or precautions  2.	The manufacturers suggested operating procedures  3.	Conditions of the load required for operation of the lifter, such as, but not limited to balance, surface cleanliness, flatness, bending, and load thickness.  4.	Not exceeding the rated load of the lifting device nor the capacity of the hoisting equipment by the combined weight of the load, lifting device, and rigging

29 CFR 1904.29(b)(1):     29 CFR 1904.29(b)(1):  A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation.    On or about March 15, 2012 the employer did not provide the required injury or illness descriptions on the OSHA 300 Log for the calendar year 2009.    a.	On or about February 27, 2009, a recordable injury occurred when the drill an employee was using broke resulting in a laceration to the middle finger of his left hand from the stainless steel he was drilling through. The description in Column F did not describe the object that directly injured the employee.    b.	On or about May 22, 2009, a recordable injury occurred when an employee was moving a large piece of metal with a co-worker, resulting in a strain to the back.  The description in Column F did not describe the object the directly injured the employee.

On or about February 18, 2010, a recordable injury occurred when an employee was filing a metal part resulting in a laceration to left hand from a sharp burr on the part he was filing.  The description in Column F did not describe the object that directly injured the employee.    b.	On or about May 4, 2010, a recordable injury occurred when an employee bumped his arm, resulting in a laceration to the right forearm.  The description in Column F did not describe the object the directly injured the employee.    c.	On or about June 10, 2010, a recordable injury occurred when an employee plugged a grinder in with the switch in the on position, resulting in a laceration to the right ring finger.  The description in Column F did not describe the object that directly injured the employee.    d.	On or about July 12, 2010, a recordable injury occurred when an employee was moving/flipping steel parts, resulting in a fracture to the ring finger and a laceration on the little finger of the right hand.  The description in Column F did not describe the object that directly injured the employee.    e.	On or about July 15, 2010, a recordable injury occurred when an employee was drilling a hole in a carbon frame and the drill bit broke, resulting in a laceration to the left middle finger.  The description in Column F did not describe the object that directly injured the employee.    f.	On or about July 30, 2010, a recordable injury occurred when an employee was using a hammer to install a ring on a metal duct.  A piece of metal broke loose resulting in a contusion to the right side of the jaw.  The description in Column F did not describe the object that directly injured the employee.

The employee tripped over a box of bolts, resulting in a fracture and sprain to the left wrist.  The description in Column F did not describe the object that directly injured the employee.    b.	On or about April 15, 2011, a recordable injury occurred when an employee was using an overhead crane to flip a stainless steel part.  The part slipped off the table, resulting in a contusion to the left thigh.  The description in Column F did not describe the object the directly injured the employee.    c.	On or about April 28, 2011, a recordable injury occurred when an employee was removing a clamp from a metal part.  The clamp popped off the part, hitting the employee in the head, resulting in a concussion.  The description in Column F did not describe the object that directly injured the employee.    d.	On or about May 17, 2011, a recordable injury occurred when an employee was turning off a grinder.  The grinder grabbed the employees glove, resulting in a laceration to the left index finger.  The description in Column F did not describe the object that directly injured the employee.    e.	On or about July 20, 2011, a recordable injury occurred when an employee was de-burring a part with a grinder, resulting in a laceration between the ring finger and little finger on the right hand.  The description in Column F did not describe the object that directly injured the employee.    f.	On or about August 9, 2011, a recordable injury occurred when an employee was pulling a piece of metal off a skid, resulting in a laceration to the right index finger.  The description in Column F did not describe the object that directly injured the employee.    g.	On or about September 2, 2011, a recordable injury occurred when an employee was loading a conveyor into a trailer.  The employee lifted the conveyor, resulting in a tear to the left bicep tendon.  The description in Column F did not describe the object that directly injured the employee.    h.	On or about October 13, 2011, a recordable injury occurred when an employee was swinging a hammer, resulting in a strain to the right shoulder.  The description in Column F did not describe the object that directly injured the employee.
