The top of the ladder used by employees to access the roof protruded through the rope guardrails forcing employees to maneuver over the ropes.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b): Employees conducting residential roofing activities were exposed to fall hazards of approximately 8 feet. The employees installing shingles to the roof were not protected from falling at the gable ends of the roof. Guardrails along the leading edge were not standard in that the top rail was rope and sagged below 39 inches. THE REUBEN CHUPP D.B.A. SCHROCK BUILDERS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310801386, CITATION NUMBER 1, ITEM NUMBER 3a, AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 8, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 204 DOUGLAS DRIVE, TUSCOLA, IL 61953.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): Employees conducting residential roofing activities were exposed to fall hazards of approximately 8 feet. The employees installing shingles to the roof were not protected from falling at the gable ends of the roof. Guardrails along the leading edge were not standard in that the top rail was rope and sagged below 39 inches. The jobsite was not inspected by competent person in that the foreman was unable to recognize the hazards onsite. THE REUBEN CHUPP D.B.A. SCHROCK BUILDERS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.20(b)(2) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310801386, CITATION NUMBER 1, ITEM NUMBER 1b, AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 8, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 204 DOUGLAS DRIVE, TUSCOLA, IL 61953.
