29 CFR 1910.178(a)(5): Where powered industrial trucks were equipped with front-end attachments other than factory installed attachments, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered:    On March 13, 2012, and at times prior thereto, where six foot extensions and a boom attachment were used for the forklifts, the forklifts did not have a name plate to indicate the truck's lifting capacity.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used for purposes prohibited by subparagraphs (a) through (f) of this paragraph:    On March 13, 2012 and at times prior thereto, in the following locations, extension cords were being used in lieu of permanent wiring;    A. East loading dock: To energize refrigerators during trouble shooting.  B. HVAC/Equipment Shop, Lunch Area: To energize a fry-o-lator.  C. Electrical Room B231: To energize the Wireguard System for the Community Living Center.  D. Chemistry Lab, Rm 1242: One extension cord energized the OceanAire Portable AC Unit and another energized a UPS battery back up system.      The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(1)(iv)(A) which was contained in OSHA Inspection Number 315053777, Citation Number 01, Item Number 006, issued on 03/07/2012, with respect to a workplace located at 5901 E 7th Street, Long Beach, CA 90822, which became final order on 03/29/2011.
