29 CFR 1926.405(g)(2)(iii): Flexible cords were not used only in continuous lengths without splice or tap. Hardservice flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. The flexible cords of several strings of temporary work lights were spliced to cords with molded plugs on them.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: A Hubbell repair plug was installed on the end of a string of temporary lighting and the flexible cord was not secured in the strain relief of the plug.

29 CFR 1926.405(a)(2)(ii)(C): Receptacles for uses other than temporary lighting were installed on branch circuits which supply temporary lighting. Receptacles shall not be connected to the same ungrounded conductor of multiwire circuits which supply temporary lighting. Temporary lighting in the basements of each apartment unit were plugged into circuits used for other purposes.
