Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. (a) Location - Platform above C4 windup batching station: On or about 2/22/12, the employer failed to provide fall protection on an open-sided platfrom 4 or more feet above the floor.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. (a) Location - C4 Batch Windup Station: On or about 2/22/12, the employer failed to adequately guard of a series of rollers used to windup cured vinyl sheets.
