29 CFR 1926.451(g)(4)(xv):     Cross bracing being used as a mid rail was not between 20 inches (0.5m) and 30 inches (0.8m) above the work platform:      a)  East side of building, northeast corner, on or about 3-14-12:  Employee were working on a scaffold system where the cross braces were used as a complete guard rail system, exposing employees to a fall hazard of up to 33 feet 6 inches.

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    a)  West side of building, south side of entry canopy, on or about 3-14-12:  One employee was working in an articulating, extensible boom, aerial lift, without his safety harness connected, exposing the employee to a fall hazard of 16 feet 2 inches.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a)  East side of building, northeast corner, on or about 3-14-12:  Employee were accessing a scaffold system without a safe means of access, exposing employees to a fall hazard of 40 feet.
