Workers were exposed to an approximate 14-17 foot fall hazard as they were on the roof performing roofing activities without the use of any fall protection systems and workers were exposed to struck-by hazards as they were using nail guns and circular saws without eye protection.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Employees were not protected by any fall protection systems while performing roofing activities on an approximate 4:12 pitch roof.  The eave to ground height was approximately 14 feet along the south side and greater than 17 feet from the roof ladder access point to the bottom of the basement stairs.
