OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to serious multiple injuries or death from being struck by falling vehicles:    ESTABLISHMENT:  The employees, performing service and repair of motor vehicles that were hoisted up by auto lifts, were exposed to the hazard of being struck by falling motor vehicles at the following bays.    A) Bay 3; Rotary Lift, Model #SPOAO-202.     B) Bay 5; Rotary Lift, Model #SPOA9A300.

29 CFR 1910.253(b)(2)(ii):  Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces were located where cylinders could be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.       ESTABLISHMENT:  An oxygen compressed gas cylinder, stored adjacent to a passageway was subject to being knocked over or displaced; an acetylene cylinder had been stored next to a 55 gallon drum of oil.

29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    ESTABLISHMENT:  Oxygen and acetylene compressed gas cylinders were stored 5 feet 6 inches apart.
