The employer did not ensure that the guard was interlocked or required a tool to remove on the AB Dick printing press.

The employer was using extension cords to power the Rico copy machines.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     Location:  In the printing press area of Mark's Quick Printing at 9567 Page Ave., St. Louis, MO  63132.    The employer did not develop a written Hazard Communication Program that included a list of hazardous chemicals at the site and that outlined the following:  1.) How outlining how employees would be trained  2.) How secondary containers would be labeled  3.) How MSDSs would be obtained and made available to workers  4.) How workers would be informed on non-routine tasks
