For the balers, grinders, shredders, and other equipment, the lockout program did not include specific machine details on lockout, including specific location on energy isolation devices (and if multiple locations, which device to use under what circumstances), specific shutdown procedures as applicable, application of a lock by each employee involved with lockout (that is, if more than one employee is performing lockout on the same machine at the same time, then more than one lock is to be applied), and the specific method on how to test the equipment to ensure it is locked out. Abatement documentation is required for this violation.
