Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. Job site located at 9944 S 136th, Omaha, NE 68138-Employees were allowed to use a Cat forklift in the service department that had holes drilled in the forks of the lift, and did not have prior written approval from the manufacturer to have the holes in the forks of the lift.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with amaximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work restshall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion. Job site located at 9944 S 136th, Omaha, NE 68138-Employees were allowed to use a Dayton Bench Grinder where the work rests were not adjusted to one eight of an inch from the stone or brush. The model number is 1Z707Y. The hazard was corrected while CSHO was on the site.

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment shall contain an equipment grounding conductor. Job site located at 9944 S 136th, Omaha, NE 68138-Employees were allowed to use a portable fan to cool the work area where the electrical plug had the ground prong removed from the plug. Received documentation from the employer that the plug on the fan had been replaced with a plug with the ground prong.
