On or about March 16, 2012 - at the above addressed jobsite, employees were performing tuck-pointing and painting operations from a tubular welded frame scaffold with a height to base width ratio greater than four to one (>4:1) that was not restrained from tipping, thereby exposing employees to falls greater than twenty five (25) feet.
