29 CFR 1910.147(c)(6)(ii):     The employer did not certify that the periodic inspections have been performed:    a. Where a lockout program had been established for equipment such as, shears, benders, and other equipment, there was no certification that annual inspection/audit of the program had been conducted.  (Note: the annual inspection is conducted to ensure that specific procedures are current, that employees are locking out according to those procedures and as required, and that equipment is available to lockout for all types of energy sources, or that cord and plug equipment is unplugged before maintenance work is performed, such as changing blades or dies, or other work.)    Abatement documentation is required for this violation.
