29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork: a. During work at the Dollar General building, Highway 100, New Haven, on March 13, 2012, an employee worked from a man basket that was not secured to the forks. (Note: there was no midrail or similar guard on both ends of the custom made man basket. There was no engineered attachment point for a lanyard.) Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift (Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system): a. During work at the Dollar General building, Highway 100, New Haven, on March 13, 2012, an employee working from a man basket supported by a Skytrack lift, was not wearing a fall protection harness. (Note: there was no midrail or similar guard on both ends of the custom made man basket. There was no engineered attachment point for a lanyard. Most lanyards are not designed to be wrapped around structure and tied back to lanyard webbing.) Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

During work at the Dollar General building, Highway 100, New Haven, on March 13, 2012, where employees were working on the roof completing seem and parapet installation, with fall hazards at 14 to 17 ft, and where the roof dimensions were 70 by 130 ft, no warning line (or any other acceptable fall protection control) was installed to warn employees of their proximity to the roof edge. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
