29 CFR 1910.178(p)(1):     If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.    On or about March 1, 2012 at Laclede Chain Company, employees were exposed to a struck by hazard in that a powered industrial truck did not have an audible back up alarm.  The powered industrial truck is operated in the induction furnace area. The truck is to be inspected daily and was not taken out of service with this defect in the safety device.

29 CFR 1910.215(a)(4):     Work rests. On offhand grinding machines, work rests shall be used to support the work. They shallbe of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with amaximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work restshall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    On, or at dates prior to, March 1, 2012, employees were exposed to caught in, or struck by, hazards from a Midaco Bench Grinder, in the Maintenance Shop, which the work rest was not properly adjusted.
