29 CFR 1910.151(c):     Where the eyes or body of any person may be exposed to injurious corrosive materials, suitablefacilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.[63 FR 33450, June 18, 1998]    At the site, while drilling for oil or reworking wells, drillers and helpers use drilling muds, which contain caustic ingredients. The presence of these caustic ingredients, and the absence of readily available eye wash stations could potentially cause eye damage.

29 CFR 1910.134(c)(2)(ii):     In addition, the employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks).    At the site, the employer did not provide medical evaluation where employees occasionally wear respirators on a voluntary basis. This condition could cause a worker who is not medically capable of wearing a respirator to have medical problems while wearing a respirator.

29 CFR 1910.134(c)(2)(i):     An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"); and    29 CFR 1910.134 App D:     1910.134 App D - (Mandatory) Information for Employees Using Respirators When not Required Under Standard.    At the site, the employer allows voluntary use of respirators without providing a copy of Appendix D to employees.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    At the site where oil wells are pumping and no guarding is present to prevent accidental entanglement or pinching from rotating parts.

29 CFR 1910.212(b):     Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving.    At the site in the machine/welding shop where a Jet Drill Press, Model JDP17MF, with holes in its base for anchoring, was not anchored to the floor, exposing employees struck by hazards and crushing hazard.

29 CFR 1910.215(a)(4):     Work rests.Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    At jobsite, employees used an 8 inch Jet Bench Grinder with the opening between the work rest and wheel that exceeded one- eight inch, exposing employees to amputation hazards.

29 CFR 1910.1200(g)(10):     Material safety data sheets may be kept in any form, including operating procedures, and may bedesigned to cover groups of hazardous chemicals in a work area where it may be more appropriate to address the hazards of a process rather than individual hazardous chemicals. However, the employer shall ensure that in all cases the required information is provided for each hazardous chemical,and is readily accessible during each work shift to employees when they are in in their work area(s).    At the site, the employer did not provide Material Safety Data Sheets when employees work with materials containing hazardous substances, exposing employees to harmful substances.
