29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) Moyer Construction on or about March 16, 2012, at the address of 212 Fremont Ave. Sandusky, OH: The employer did not ensure an employee was protected from falls while installing new roofing material to a steep sloped roof on commercial structure. The employee was exposed to falls of up to 8ft to the ground below.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. a) Moyer Construction on or about March 16, 2012, at the address of 212 Fremont Ave. Sandusky, OH: The employer did not ensure the side rails of the 16 foot fiberglass ladder were extended at least three feet above the landing. An employee was exposed to falls while using the ladder to access the working level to perform roofing activities.
