29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. The interlocks on the main guard doors to the Mazak NEXUS 510C-II and the Mazak NEXUS 700D-II CNC machines were non-operational. Employees were exposed to the hazard of contacting the rotating cutters during normal operations and/or the moving table during set-up operations.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. In the main shop at the workstation for the Vilh. Pederssen Tool Grinder machine the pulley drive mechanism was not guarded. Employee/s were exposed to a caught-between hazard.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: At the OKUMA MC-4VA CNC Machine and in the parts cleaning room employees were allowed to utilize compressed air at the unrestricted plant air pressure of approximately 115 psi for cleaning purposes. Employees were exposed to the hazard of injecting compressed air into their body.
