(a)	RPMX1000, 90 S. 18th Ave., Brighton, CO 80601:  On and before March 16, 2012, the employer did not protect his roofing employees as they worked without head protection in areas where roofing operations were taking place at higher levels. 29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a)  	RPMX1000, 90 S. 18th Ave., Brighton, CO 80601:  On or before March 16, 2012, the employer did not protect his roofing employees from fall hazards as they worked without fall protection. Four employees were conducting roofing operations from Genie aerial lifts and were not wearing fall protection. This hazard exposed the employees to a fall of  30 to 40 feet.    Abatement Note: A personal fall arrest system is recommended with lanyard attached to the boom or basket when working from an aerial lift. As of January 1, 1998, Subpart M of the 29 CFR 1926 part 1926.502(d) provides that body belts are not acceptable as part    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.454(a):  The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure(s) to control or minimize those hazards:  (a)	RPMX1000, 90 S. 18th Ave., Brighton, CO 80601  On and before  March 16, 2012, the employer did not have each employee who performs work while on a aerial lift(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of aerial lift(s) being used and to understand the procedure(s) to control or minimize those hazards. Employees applied roofing materials while working from a Genie aerial lift. This condition exposed the employees to a fall hazards and electrical hazards due to not recognizing the hazards associated with working on the aerial lift.   Abatement Note: The training shall include the following elements at a minimum:  (1)  The nature of any electrical hazards, fall hazards, and falling object hazards in the work area;  (2)  The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used:  (3)  The proper use of the scaffold, and the proper handling of materials on the scaffold;  (4)  The maximum intended load and the load-carrying capacities of the scaffold used; and  (5)  Any other pertinent requirements of this subpart.
