29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:    Jobsite located at 349 Main Street, Somers, Connecticut:  The company had not implemented a safety and health program that included frequent and regular site inspections in that:    A.  Employees were engaged in leading edge work without fall protection systems provided.    B.  Floor and wall openings were not adequately protected.    C.  Adequate ladder(s) were not provided to access upper levels.    D.  Employees were required to install roof sheathing in windy conditions exceeding 40 mile per hour gusts.    E.  Adequate electric shock protection was not provided where temporary wiring was used.

Jobsite located at 349 Main Street, Somers, Connecticut, Second Floor of Arcade Building:  Fall protection was not provided on the following wall openings:    A.  Exterior window openings.  B.  Wall stud openings were the stud spacing exceeded sixteen inches on center.
