29 CFR 1926.307(b)(1): Flywheels located so that any part is 7 feet or less above the floor or platform were not guarded in accordance with the requirements of this subparagraph: a) Elevators Unlimited, Inc. at 4455 E. 12th Ave., Denver, CO 80220: On or before March 8, 2012, the employer did not ensure that an employee was protected from a flywheel located within 3 vertical feet of the landing above the elevator shaft, used as a part of the mechanical power-transmission of the elevator. Condition exposed this employee to amputation hazards.

29 CFR 1926.307(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs (m) and (o) of this section: a) Elevators Unlimited, Inc. at 4455 E. 12th Ave., Denver, CO 80220: On or before March 8, 2012, the employer did not ensure that an employee was protected from two operational pulleys located immediately above the landing above the elevator shaft, and which were used as a part of the mechanical power-transmission of the elevator. Condition exposed this employee to amputation hazards.
