29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Northwest end of the bridge: On or about March 5, 2012, employees engaged in the installation of a metal guardrail system on a highway bridge apporach were not wearing protective helmets (hard hats). This exposed them to the hazard of being struck in the head by guardrail sections.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: Northwest end of the bridge: On or about March 5, 2012, employees engaged in the installation of a metal guardrail system were not wearing protective eyewear. The employees were exposed to the hazard of being struck in the eyes by debris generated by power tool operations.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Bridge sides: On or about March 5, 2012, and times prior to employees rebuilding a highway bridge concrete and metal guardrail system were not provided with fall protection. This exposed the employees to the hazard of falling over 6 feet.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: Northwest corner of the bridge: On or about March 5, 2012, employees engaged in the rebuilding of a highway guardrail system used electric hand tools to install the metal guardrails. The handtools were powered from a portable generator with no form of ground fault protection provided.
