29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about March 15, 2012, an employee gained access to the roof of a building through the use of a Werner 8 foot a-frame ladder. The ladder was in the closed position and leaned against roof edge. The ladder was not being used according to the manufacturer instructions.
