Employees were exposed to potential injurious corrosive materials.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    On or about February 28, 2012, at Collin D#4 Well , Midland TX, Sendero Drilling Rig #8 had a suction pit motor with a broken guard. The motor was being used to run the mixing pump. Employees were exposed to potential amputations.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    On or about February 28, 2012, at Collin D#4 Well , Midland,TX, Sendero Drilling Rig #8 tool pusher's living quarters trailer home and the fresh water tank were not properly grounded. The trailer home ground rod was barried 2 inches into the ground and the fresh water tank ground rod was not barried at all. This conditions did not allow a pemanent and cotinuous path to ground from circuits, equipment and enclosures.  Employees were exposed to potential electrical hazards.
