29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use.  On or about March 10, 2014, and at times prior thereto, the employer had not developed a written respiratory protection program with worksite specific procedures for mud operators to identify the proper use of respirators when mixing chemicals.  This condition exposed employees to health hazards.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.  On or about March 10, 2014, and at times prior thereto, employer did not evaluate the respiratory hazard for the mud operators that included but not limited to personal air monitoring, or reasonable estimate of employee exposure to corrosive chemicals.  This condition exposed employees to respiratory health hazards.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting face piece respirators were not fit tested whenever a different respirator face piece (size, style, model or make) was used.  On or about March 10, 2014, and at times prior thereto, employees were using N-95 respirators without first being fit tested for that type of respirator.  This condition exposed employees to health hazards.
