Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by drill pipe:    Rig #3: On or about July 20, 2012, the employer did not ensure that personnel on the rig floor and the personnel located next to the pipe catwalk below followed the Employer's own procedures for safe handling of tubulars while lowering drill pipe through the V-door down to the catwalk. This condition exposed an employee to struck-by hazards from moving/descending drill pipe.       Among other methods, one feasible and acceptable abatement method to correct the hazard would be to follow Employer's procedures for safe handling of tubulars.      Another method would be to use a laydown machine to allow for safe movement of pipe without exposing employees to struck-by hazards from falling pipe.
