29 CFR 1910.23(a)(8):     Every floor hole into which persons can accidentally walk were not guarded:    (a) Squeezer Line:  Floor holes through which conveyor belt runs and used mold sand is processed, is not protected from employees stepping into the hole. This exposed employees to caught-in and in-going nip point hazards.

29 CFR 1910.23(c)(3):     Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.    (a) Squezzer Line: Didion shaker and rotating drum through which used sand is recovered, was not guarded from employees falling into the recessed floor area and onto the rotating drum and vibrating equipment. This exposed employees to caught-on, caught- in and amputation hazards.

29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) A Hazardous Energy Control Program was not developed to ensure that employee(s) who perform maintenace and repair on production equipment, such as but is not limited to the Didion, shakers, molders, melting pots, belt grinders, and local exhaust system, are not exposed to unexpected energizing, startup or release of stored energy from mechanical and electrical components.

29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points:    (a) Plamer Line: Baldor belt grinders 1 through 6 did not have the ingoing nip points created by the grinding belt and pully guarded, and the spindle ends and nuts guarded exposing operators to a caught-in, caught-on and amputation hazards.    (b) Dizza Line: Baldor belt grinders 1 through 6 did not have the ingoing nip points created by the grinding belt and pully guarded,  and the spindle ends and nuts guarded, exposing operators to a caught-in, caught-on and amputation hazards.    (c) Brass Line: Baldor belt grinders 1 and 2 did not have the ingoing nip points created by the grinding belt and pully guarded, and the spindle ends and nuts guarded, exposing operators to a caught-in, caught-on and amputation hazards.    (d) Squezzer Line: Didion infeed conveyor belt did not have the ingoing nip points created at the roller wheel tensioning point guarded, exposing employees to a caught-in, crushing hazard and amputation hazard.
