29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, a safety net system, or a personal fall arrest system:     Location: 41 Chatworth Ave Larchmont NY 10538    On or about 3/15/12,  a) Two 9.5 ft X 9 ft holes for the future stairway did not have adequate guarding around it. Only one guardrail was provided at approximately 36 inches above the walking working surface.     b) Several wall openings did not have any type of fall protection.
