29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) MIDO Printing Co., Inc. at 6845 East 48th Ave., Denver, CO 80216: On or before March 9, 2012, the employer did not ensure that employees were protected from in going nip points and rotating parts in the following instances: 1) The Miller Printing Company Unimmatic C3 serial number A20828 included exposure to rotating parts, including gripper fingers, on the lower duplexing rollers without adequate machine guarding; 2) The Miller Printing Company Unimmatic C3 serial number A20828 included exposure to ingoing nip points on the upper initial feed rollers without adequate machine guarding; and 3) The Miller Printing Company Unimmatic C3 serial number A20828 included exposure to rotating parts and ingoing nip points where the side shields were easily defeated. Condition exposed this employee to amputation hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) MIDO Printing Co., Inc. at 6845 East 48th Ave., Denver, CO 80216: On or before March 9, 2012, the employer did not conduct periodic inspections of the energy control procedures at least annually to ensure the that the procedures and the requirements of the standards were being followed. Condition exposed employees to amputation hazards. Abatement note: 1926.147(c)(6)(ii) provides that the employer shall certify that the periodic inspections have been performed.
