29 CFR 1910.176(b):     Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    Location: Warehouse    On or about 3/16/12, employees working in the warehouse were exposed to overhead struck by hazards in the following areas;    1. Rack W3SCARF-3  2 boxes of scarfs weighing approximately 80-pounds stored on top of another box on a shelf approximately 9-feet in height was leaning inwards towards the aisle.  2. Rack W3A401-3  10 boxes of reversible vests stored on top of other boxes approximately 12-feet in height was leaning inwards and towards the aisle.  3. Rack W2A207-2  4 boxes of petal shirts stored on top of other boxes approximately 10-feet in height was leaning inwards and towards the aisle.  4. Rack W2A5-2  23 boxes of miscellaneous clothing stored on top of a shelf that was bent towards the middle of the shelf and stored approximately 13-feet in height was leaning inwards towards the aisle.  5. Rack W1A1-4  the vertical upright of the rack stacked with boxes of merchandise was bent and twisted upright.

29 CFR 1910.303(b)(7)(i):     Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:    Location: Warehouse    On or about 3/16/12, the employer did not install the faceplate cover of the electrical outlet and light switch box that supplied power to two outlets and a light switch.  The 110-volt internal wires exposed employees who turn on and off the lights to the warehouse and that use the two extension cords to electrical shock hazards.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Location:  Dock Area    On or about 3/16/12, an extension cord running along the concrete floor was damaged and frayed exposing the internal wires of the cord.  The employer did not do visual inspections of extension cords.
