29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment was performed through a written certification:     (a)	AG Masonry Corp @ 600 Chestnut St., Leadville, CO 80461: On and before 3/20/12, the employer did not maintain a written certification of a workplace hazard assessment. Employees are exposed to hazards such as, but not limited to, glues (Pro Series Polyester Stone Adhesive), solvents (Acetone), and flying debris. The employer requires employees to wear personal protective equipment such as, but not limited to, eye protection, hearing protection, steel toed foot protection, and splash aprons during granite countertop fabrication.

The forklifts are used to transport granite slabs.
