29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: On or about March 20, 2012, in the facility, employees were required to service and maintain machines and equipment, including, but not limited to, tractors and forklift trucks. The employer did not conduct a periodic inspection of each energy control procedure at least annually to ensure that the procedure and the requirements of the standard were being followed.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about March 20, 2012, at the Repair/Maintenance Shop, employees (mechanics) required to service and maintain tractor trucks did not affix a tagout device to the energy isolating device (ignition control). Employees hung a red "Out of Service" tag to the steering wheel or door. Employees were thereby exposed to the hazards associated with the unexpected energization of moving parts.
