29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. At the job site, 1119 Ratone, Manhattan, Kansas, employee was operating a masonry power saw without wearing eye and face protection. Employee was exposed to struck by hazards while operating masonry cutting saw.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: At the job site, 1119 Ratone, Manhattan, Kansas, employees were working from the 2nd level of a framed scaffold without having top and medium guardrails in place at the north and south ends of the scaffold. Employees are exposed to fall hazards of approximately 13 feet to the ground.
