29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, or they were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts:    On or about 3/15/12    Location: Shop, 1364 Inwood Avenue, Bronx, NY 10452    a) A homemade 10' 2" long fork extension consisting of a 6" x 4"  "I" beam was placed on the Clark forklift SN# 69328275-49. The original forks were 4' long     NOTE: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:    On or about 3/15/12    Location: Shop, 1364 Inwood Avenue, Bronx, NY 10452    a) The protective guard was missing from the left side abrasive wheel on the Teg-Fit 6 inch bench top grinder exposing the spindle nut. 29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    On or about 3/15/12    Location: Shop, 1364 Inwood Avenue, Bronx, NY 10452    a) The left work rest on the TegFit 6 inch bench top grinder was missing.    b) The right work rest on the TegFit  6 inch bench top grinder was bent and 3/4 inch away from the face of the abrasive wheel.     NOTE: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked in a conspicuous place in each space to which they related:    On or about 3/15/12    Location: 1364 Inwood Avenue, Bronx, NY 10452    a) A office area was framed out in the corner of the open shop area. The office was not part of the original structure and had a flat roof/ceiling. There was no sign indicating the maximum load that could be placed on the ceiling/roof. Tubular welded scaffold frames were being stored on the roof/ceiling.
