29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    JOB SITE: 9281 Forman Road, St. Louis, MO 63123, at this work site, three (3) workers were observed actively removing old roof shingles on a residential 3:12 pitch roof, approximately 8 feet 11 inches eave to ground level, and were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  The workers were exposed to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    JOB SITE: 9281 Forman Road, St. Louis, MO 63123, the employer failed to provide a training program for his workers who were exposed to fall hazards and allowed his workers to remove old roof shingles, on a 3:12 pitch roof, approximately 8 feet 11 inches eave to ground level without any form of fall protection.  The workers were exposed to a fall hazard.
