The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wore eye protection that incorporates the prescription in its design, or wear eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    a) Building Supplies-Right Rear of Store- On or about 20 March 2012- CSHO observed employee operating panel saw without wearing goggles or other protective eyewear over his prescription eyeglasses. Employee was exposed to flying object or eye injury hazards.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment    a) Front of store, Electrical panelboxes, on or about 20 March 2012- Employees were exposed to shock/electrocution hazard due to panel box "IS2" was missing 2 breakers and appropiate covers were not installed to protect the openings.    b) Front of store, Electrical panelboxes, on or about 20 March 2012- Employees were exposed to shock/electrocution hazard due to panel box "R" was missing 1 breaker and the appropiate cover was not installed to protect the opening.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  Note:  Applies to any extinguishers provided in the workplace.  Maintenance check includes an internal examination, except extinguishers with stored pressure.    a) On or about 20 of March 2012, at the worksite:  Seventeen(17) out of a total twenty nine(29) fire extinguishers were found to have expired annual maintenance checks as of February 2012. The specific locations of each are as follows:  Restroom Entrance,Training Room/Break Room Entrance,Training Room, Break Room, Aisle 24, Backwall Plumbing Fixtures, Backwall Outdoor Light Fixtures, Aisle 35, Aisle 8, Mill Works Desk, Aisle 13, Front End Center, Paint Desk, Carpet Sales Desk, Receiving Entrance, Receiving Exit Door, and Aisle 42.
