29 CFR 1910.178(l)(3):     Training program content. Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace:      29 CFR 1910.178(l)(3)(ii)(I):  Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation:        a.     On the site where logging operations were performed, the employers training program lacked effective and complete training content on the operations during logging and did not contain specific information as to the safe operation of, or around, a skid steer loader, thereby exposing employees to a struck-by hazard.

29 CFR 1910.266(d)(6)(iii):     Each employee performing a logging operation at a logging work site shall work in a position or location that is within visual or audible contact with another employee.    a.     On the site where trees were harvested in the woods, employees were working in the yard to move logs while another employee was in the woods cutting trees.  The tree cutting employee was not within site or audible contact of the employees working in the yard, thereby exposing the employee to a struck-by hazard.

29 CFR 1910.266(i)(7)(i): The employer shall assure that each employee, including supervisors, receives or has received first-aid and CPR training meeting at least the requirements specified in Appendix B:    a. On the site where employees were harvesting trees, the employer did not ensure employees were provided first-aid/CPR training in the event of an accident, thereby exposing employees to a serious injury or death.
