29 CFR 1904.10(a): The employer did not record on the OSHA 300 Log each case of occupational hearing loss, in which an employee's audiogram revealed a standard threshold shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 dB or more above audiometric zero in the same ear(s) as the STS (averaged at 2000, 3000 & 4000 Hz): a) The results of annual audiometric testing of employees' hearing conducted on 4/25/2011 disclosed 3 cases which met the recordability criteria for occupational hearing loss and were not entered on the 300 Log for 2011. b) The results of annual audiometric testing of employees' hearing conductied on 5/11/2010 disclosed 1 case which met the recordability criteria for occupational hearing loss and was not entered on the 300 Log for 2010. Condition(s) noted on or about 3/21/2012.
