29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: On or about March 6, 2012, and times prior thereto, southwest side of construction site, employees of a contractor were working in a trench that was 8 feet deep. The competent person did not conduct an inspection of the trench before allowing employees to enter. Employees were exposed to the hazard of being struck/crushed by falling rock and soil.

Each employee who entered the trench was not protected from cave-in by an adequate protective system. Employees were exposed to the hazard of being struck/crushed by falling rock and soil.
