29 CFR 1910.132(a):     Protective equipment, including personal protective equipment for the eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used and maintained in a sanitary and reliable condition whereever it was necessary by reason of hazards of processes or enviornmental, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    Fall protection was not provided for employees who have to climb on top of the mold machines which are approximately 6 to 15 feet high to check sensors.

29 CFR 1910.132(d)(2):     The employer did not verify that the required workplace hazard assessment had been performed through written certification that identified the workplace evaluated; the person certifying that the evaluation had been performed; the date(s) of the hazard assessment; and, identified the document as a certification of the hazard assessment:    The required workplace assessment was not written and certified as being performed.    Employees who are required to check sensors on mold machines which are approximately 6 to 15 feet high, were not provided fall protection.    The employer shall:    (a)  Select, and have each affected employee use the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;    (b)  Communicate selection decision to each affected employee; and,    (c)  Select PPE that properly fits each affected employee

29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The company did not establish a program to protect employees involved with servicing and maintenance of machines and equipment from the unexpected energizing, start up or release of stored energy.  Energy sources, included, but are not limited to, electrical, pneumatic, hydraulic, etc.  The company maintains and servides equipment such as, but not limited to, plastic injection molding machines, etc.
